Required Documents

Every player, coach, scorekeeper and clock worker and their parents or legal guardians must read and sign the following forms:  Registration form, conduct forms, waiver and release form. The signatures on the form will constitute an acknowledgment that each person has read and understands this form in its entirety.  All parents are responsible for paying registration fee and participating in fundraisers.  Parents should provide coaches with a COPY of childs birth certificate and report card.
